- ID people impacted by the change, how they are impacted and their attitude toward the change
- ID leaders of the change (e.g., by PSL, by work group) and evaluate attitude toward the change
Create Change Management Plan
- Select strategies & tools to manage change risk and support stakeholders and leaders
- Develop change management plan activities and timelines to align with program plan

Develop Communication Strategy and Plan
- ID effective channels, overall approach, details for who, what, when, why, how
- Create key message(s) and supporting materials
Develop Sponsor Action Plans
- Educate leaders about their role during each phase of the project
- ID specific actions for leaders in each project phase
Develop Change Network and identify ways for Change Network to provide input to/participate in the program
Develop Training Plan, Curriculum and Materials
- Determine the skills employees will need
- Develop training curriculum and training materials
Execute Plans
- Perform activities per plans
- Monitor effectiveness and review/revise activities as needed
Develop Transition/Sustainability Plan

Conduct Change Readiness Assessment to
determine whether or not people are ready to “go live” and ID mitigation plans
Deliver Training to educate stakeholders about how to perform their jobs in the future, providing refreshers as needed and supporting learning aids

Develop Resistance Management Plan (on-going post-launch)
- ID areas of resistance in the organization, the probable source and implement actions to manage the resistance
